Registration Process

Download and fill out all "9" forms listed below highlighted in "RED". Scroll down to Part 2 and complete the Online Registration, submit and print a copy of it. Return the completed 9 forms, a copy of your Online Registration and copies of any additional required documents including the application fee to Mrs. Mason (in person, unless living out of town) in the St. Thomas the Apostle school office. (Making copies of the forms is the applicant's responsibility.) Incomplete applications will not be accepted.
Testing / Interviews will be scheduled after a completed application is submitted.
Student Recommendation Letter(s) from current teacher(s), including pre-school teachers are confidential and need to be faxed to 602-381-3256 prior to submitting your application.

Please note that if you are a member of St. Thomas the Apostle Church, the parish verification form (#9) stays with the application packet. You are only to fill out the top portion. We will submit these as a group to Father John for his portion and signature. If you belong to another parish, please turn the completed form with your Pastor's signature in with your packet. If you do not attend church or are not interested in the discount, just put N/A on the form.

Part 2 Online Registration
20010–2011 Registration Schedule for Returning Families
- January 6, 2010 — Re-Registration Questionnaire for current students in Wednesday packet
- January 8, 2010 — Re-Registration Questionnaire due for current students
- January 13, 2010 — Applications distributed in Wednesday packet for siblings of currently enrolled STA students
- January 19, 2010 — Sibling Applications due to school office
2010–2011 Registration Schedule for Prospective New Families
All new students in grades K-8 will be tested before acceptance. All new students/parents (and or guardian) in grades 2-8 will be interviewed by the administration.
- January 21, 2010 — 8:30 AM, Open House in the Parish Hall for prospective new families
- January 27, 2010 — New Family Applications accepted 7:00 am. School Office.
Required Documents to Submit with Your Application
All families with new students must submit COPIES of the following applicable forms in order for your application to be considered. Copying is the applicant's responsibility and originals, if submitted, will not be returned or photocopied.
- Birth Certificate (State certified copy)
- Recent Photo (approx. 3" x 4")
- Immunization Records (Immunizations MUST be current)
- Baptismal Certificate
- First Reconciliation Certificate (if applicable)
- First Eucharist Certificate (if applicable)
- Current Year's Report Card (if applicable)
- Past Year's Report Card (if applicable)
- Most Recent Standardized Test Scores (if applicable)
- Student Recommendation Letter(s) - From Current Teachers including pre-school teachers
- $100 Non-refundable Application Fee per student (covers testing and administrative costs)
- Any court-ordered custody visitation directive
