Hello, I am the person responsible for helping you obtain a copy of your sacramental record. To get started, please fill out the form below. After filling out the form, you will receive a confirmation email from me stating that I have received your request.
Diocesan policy requires that individuals must present valid identification to pick up certificate. Due to this requirement, you MUST pick up your certificate in person.
